To apply to have your benefits coordinated with your spouse’s benefits, download the Coordination of Benefits Form. Follow these procedures to receive the maximum amount of reimbursement: If the Expenses are Yours
Example: You require the services of a physiotherapist. The cost of your sessions totals $200. You submit your claim to Manulife and receive $160 (80% of $200). You will receive your reimbursement with an explanation of benefits from Manulife. Send that along with the necessary forms for submission to your spouse’s plan. You should be reimbursed for the remainder according to the coverage provided under your spouse’s plan. If the Expenses are For Your Spouse
